Clinical assessment of medical, cognitive and personal history and status, including evaluation of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Includes a customized Aging In Place care plan.
Foundational Services
Available as standalone options, based on your family’s needs
MSW-led discussion of values and medical wishes to inform completion of a standard Living Will and/or Durable Power of Attorney for Healthcare, compliant with current New Hampshire laws and regulations. Complementary on-site notarization is included.
Ongoing, scheduled assessment of your loved-one’s changing needs, including regular visits and follow-up communication of our findings, with guidance on addressing any identified changes.
Care Management Services
Offered exclusively to our clients who have completed the Home Living Assessment
Our highly-experienced social worker can act as a liaison with medical providers, ensure coordination of home care service options, and serve as your dedicated advocate during times of care transition.
24/72 Crisis Assessment
This premium service provides an urgent assessment of your loved one's status to allow for rapid development of a plan of action. This urgency may be appropriate in the event of a dramatic change in your loved one's health or cases of sudden care collapse due to the death or hospitalization of your loved one's sole caregiver.
Service includes Home Living Assessment conducted on-site within 24 hours and Aging in Place care plan delivered within 72 hours after assessment
Service includes Advance Care Planning, if required.
While our 24/72 service is available as a standalone option, we strongly advise that it be followed by ongoing care management.
