Our Care Management program provides you with professional assurance as circumstances change. This ongoing oversight supports both you and your loved one with the knowledge that even subtle changes will be identified before they become a crisis.
We offer multiple care management plans to meet your family where you are now, and plans can be tailored in response to your loved one's changing needs and your family's unique situation.
The frequency of care management support will depend on the intensity of your loved one's needs, as identified in the Home Living Assessment. All Care Management plans include a quarterly written update, detailing any new concerns and recommendations.
Because we understand that some concerns cannot wait for your next scheduled check-in, we offer our Care Oversight clients the option to book as-needed phone calls and in-person visits.
Pricing begins at $750/quarter and includes a written,
quarterly care plan update
