Appointments

At appointments you are unable to attend, we convey your questions and concerns to the medical provider, with the unique ability to raise follow-up questions in real time.

 

When you are in attendance, our presence ensures clinical oversight, empowers family decision-making and allows you to provide your full attention to your loved one.

Care Transitions

We serve as your loved one's dedicated advocate in discharge planning conversations.

We confirm your loved one's home environment is properly equipped prior to discharge, coordinating with durable medical equipment providers when needed.

 

We ensure all relevant information is shared with outpatient medical providers to prevent gaps in care.
